ALL ABOUT MY DAD

Happy Father’s Day! I love questionnaires. At our house we try to do them yearly. It’s fun to see how the answers change. Below are a bunch of options to choose from. Pick which one fits best!

If your child is too young to write, simply fill it in for them. If they can write, it’s even more fun to see their spelling/handwriting if younger.

Make sure to write “Dad” at the top of page. (after All About My). It looks nice if the “Dad” matches the rest of writing in the document below.

For the photo- you can place a photo into the digital copy, then print, or glue an actual photo on once you print out the document. If you don’t want to use a photo, have your child draw an illustration of them with their dad. A photo or illustration will make it extra special.

Completing this activity is fun! You’ll probably learn something new! =)

Additional Materials:

  • pencils/markers/colored pencils
  • photo of Dad (or can draw a picture)
